Controlled stationery
7.1 |
Controlled
Stationery is any stationery, which, in the wrong hands, could be used
to obtain medicines fraudulently. |

7.2 |
Stocks of the following Controlled Stationery shall be received and held secure. Stocks of the following will be distributed by the hospital pharmacy department: |

Once in the ward/unit/department in the hospital or elsewhere, the person receiving (e.g. nurse in charge) the controlled stationery is responsible for its security. |

7.3 |
A record shall be kept in pharmacy or by Primary Care Services of the receipt and issue of Prescription Forms (including prescription code indentifier ranges) and Controlled Drug Order Books, including the date issued and the identity of the person requesting and issuing it. Only one Controlled Drug order book must be held on a ward at any time except when otherwise agreed locally to meet exceptional circumstances e.g. some community hospitals. The completed book will be returned to the ward for retention for 2 years. Records shall be periodically examined by pharmacy staff for inconsistencies, and any anomalies investigated. |

7.4 |
Loss or theft of any controlled stationery shall be reported immediately to the practice/line manager, and to the Senior Pharmacist and managed and reported in accordance with the Adverse Incident Management Policy. The CD Accountable Officer must be informed. |

7.5 |
Unused Controlled Drug Order Books and Prescription Forms must be returned to the issuing pharmacy department or primary care services, where their receipt must be recorded. |

7.6 |
Records of the receipt and issue of controlled stationery will be retained securely for 2 years in the relevant clinical area. |

7.7 |
Controlled Drug registers that are being replaced must have part-used pages ruled off, and the register must be sealed in an envelope. |

7.8 |
Controlled Drugs Registers and Order Books shall be held for two years from date of last entry, in accordance with the Misuse of Drugs Regulations 2001. |
